Neglect is different. It happens when a caregiver or facility fails to provide the care a resident needs, whether through carelessness, understaffing, or poor oversight. Neglect does not have to be intentional to cause serious harm. A resident who is left in a wet bed for hours, who misses medications regularly, or who is never helped to change positions can suffer just as much from neglect as from direct abuse.

What Are the Physical Signs of Nursing Home Neglect?

Physical signs are often the most visible indicators that something is wrong. If you visit your loved one and notice any of the following, take it seriously and do not accept vague explanations from staff:

  • Bedsores, also called pressure ulcers, which develop when a resident is left in one position for too long without being moved or repositioned

  • Unexplained weight loss or signs of dehydration, such as cracked lips, dry skin, or sunken eyes

  • Poor hygiene, including unwashed hair, dirty clothing, soiled bedding, or a strong odor of urine

  • Frequent unexplained injuries, bruising, or falls

  • Infections that keep returning or are not being treated

  • Dirty or unsafe living conditions in the resident's room or common areas

Aging naturally brings some physical changes. However, rapid decline, repeated injuries, or obvious signs of poor hygiene are not a normal part of getting older. They are red flags.

What Are the Emotional and Behavioral Signs of Nursing Home Neglect?

Neglect does not only harm the body. It causes emotional suffering, too. Sometimes, the emotional signs appear before the physical ones. Watch for changes in your loved one's mood or behavior, especially if the changes are sudden or seem tied to specific staff members or situations.

Signs to watch for include withdrawal from activities or conversations they previously enjoyed, increased anxiety or fearfulness, crying more than usual, a flat or vacant expression, and reluctance to speak in front of staff. Some residents, particularly those with dementia or cognitive decline, may not be able to tell you directly what is happening. Their behavior is often the only way they can communicate that something is wrong.

If your loved one seems frightened, unusually quiet, or different in ways you cannot explain, do not dismiss it. Visit more often, ask questions, and pay attention to how staff respond when you bring up concerns.

What Are the Signs That Medical Needs Are Being Ignored in a Nursing Home?

One of the most serious forms of nursing home neglect is the failure to provide proper medical care. This can include missed or incorrect medications, untreated infections, and failure to follow a doctor's care plan. It also covers delays in getting a resident to the hospital when needed and failure to monitor conditions like diabetes, heart disease, or wounds that require regular attention.

Virginia's nursing home oversight laws were significantly strengthened following a 2025 legislative overhaul that expanded the Virginia Department of Health's authority to penalize facilities for repeated violations and increased fines for noncompliance. These changes came in part because inspections had been falling behind, leaving many residents without adequate oversight for long periods of time. If your loved one's medical condition has worsened in ways that do not match their diagnosis, or if staff cannot clearly explain changes in their health, that warrants a closer look.

What Should You Do If You Suspect Nursing Home Neglect in Virginia?

If you believe your loved one is being neglected, act quickly. The situation is unlikely to improve on its own, and delays can allow harm to continue or worsen.

Start by documenting everything you observe. Take photographs of any physical signs like bedsores, injuries, or unsanitary conditions. Write down dates, times, and exactly what you saw. Keep records of any conversations with staff or administrators, including what was said and how they responded to your concerns.

Report your concerns to the nursing home administrator in writing so there is a record.
